Good People, by Singaporean playwright Haresh Sharma, unmasks racial and religious tensions between Singapore’s increasingly diverse racial groups and the attendant ramifications on the healthcare ecosystem and the doctor-patient relationship. Drawing upon Michel Foucault’s notion of heterotopia, this paper argues that, in Good People, Sharma employs theater as a site of resistance by calling into question state and medical authority. First, state authority is challenged through the play’s scrutiny of the ideological principle of multiculturalism and its usefulness in fostering meaningful (...) cross-cultural exchanges and acceptance of different cultural and religious beliefs in the clinic. Second, the play destabilizes medical authority by surfacing the complex relationship between the doctor’s unconscious biases, racial and religious prejudice, and clinical judgment, thereby casting doubt on medicine’s claims of objectivity. In doing so, this paper argues, the play resists simplistic binary categorizations of the behaviors and motivations of the characters into good/bad or right/wrong, instead raising questions about power, knowledge, and contesting truths within the confines of the cultural space of a hospice. No special "anniversary number" was published on that occasion, but the April 1999 issue ran two commemorative articles filled with reminiscences of people and events representative of the history of the publication. On May 27, 1978, Michel Foucault gave a lecture to the French Society of Philosophy where he redefines his entire philosophical project in light of Immanuel Kant's 1784 text, "What Is Enlightenment?" Foucault strikingly characterizes critique as the political and moral attitude consisting in the "art of not being governed in this particular way," one that performs the function of destabilizing power relations and creating the space for a new formation of the self within the "politics of truth." This volume (...) presents the first critical edition of this crucial lecture alongside a previously unpublished lecture about the culture of the self and three public debates with Foucault at the University of California, Berkeley in April 1983. There, for the first time, Foucault establishes a direct connection between his reflections on Enlightenment and his analyses of Greco-Roman antiquity. However, far from suggesting a return to the ancient culture of the self, Foucault invites his audience to build a "new ethics" that bypasses the traditional references to religion, law, and science. Objective: Ethical guidelines are designed to ensure benefits, protection and respect of participants in clinical research. Clinical trials must now be registered on open-access databases and provide details on ethical considerations. This systematic survey aimed to determine the extent to which recently registered clinical trials report the use of standard of care and post-trial obligations in trial registries, and whether trial characteristics vary according to setting. Methods: We selected global randomized trials registered on http://www.clinicaltrials.gov and http://www.controlled-trials.com. We searched for intervention (...) trials of HIV/AIDS, malaria, and tuberculosis from 9 October 2004, the date of the most recent version of the Helsinki Declaration, to 10 April 2007. Results: We collected data from 312 trials. Fifty-eight percent (58%, 95% CI = 53 to 64) of trial protocols report informed consent. Fifty-eight percent (58%, 95% CI = 53 to 64) of trials report active controls. Almost no trials (1%, 95% CI = 0.5 to 3) mention post-trial provisions. Most trials measure surrogate outcomes. Twenty percent (20%, 95% CI = 16 to 25) of trials measure patient-important outcomes, such as death; and the odds that these outcomes are in a low income country are five times greater than for a developed country (odds ratio (OR) 5.03, 95% CI = 2.70 to 9.35, p = < 0.001). Pharmaceutical companies are involved in 28% (CI = 23 to 33) of trials and measure surrogate outcomes more often than nonpharmaceutical companies (OR 2.45, 95% CI = 1.18 to 5.09, p = 0.31). Conclusion: We found a large discrepancy in the quality of reporting and approaches used in trials in developing settings compared to wealthier settings. (shrink)

To fulfill their crucial duty of relieving suffering in their patients, physicians may have to administer palliative sedation when they implement treatment-limitation decisions such as the withdrawal of life-supporting interventions in patients with poor prognosis chronic severe brain injury. The issue of palliative sedation deserves particular attention in adults with serious brain injuries and in neonates with severe and irreversible brain lesions, who are unable to express pain or to state their wishes. In France, treatment limitation decisions for these patients (...) are left to the physicians. Treatment-limitation decisions are made collegially, based on the presence of irreversible brain lesions responsible for chronic severe disorders of consciousness. Before these decisions are implemented, they are communicated to the relatives. Because the presence and severity of pain cannot be assessed in these patients, palliative analgesia and/or sedation should be administered. However, palliative sedation is a complex strategy that requires safeguards to prevent a drift toward hastening death or performing covert euthanasia. In addition to the law on patients' rights at the end of life passed in France on April 22, 2005, a recent revision of Article 37 of the French code of medical ethics both acknowledges that treatment-limitation decisions and palliative sedation may be required in patients with severe brain injuries and provides legal and ethical safeguards against a shift towards euthanasia. This legislation may hold value as a model for other countries where euthanasia is illegal and for countries such as Belgium and Netherlands where euthanasia is legal but not allowed in patients incapable of asking for euthanasia but in whom a treatment limitation decision has been made. Schopenhauer’s invective is legendary among philosophers, and is unmatched in the historical canon. But these complaints are themselves worthy of careful consideration: they are rooted in Schopenhauer’s philosophy of language, which itself reflects the structure of his metaphysics. This short chapter argues that Schopenhauer’s vitriol rewards philosophical attention; not because it expresses his critical take on Fichte, Hegel, Herbart, Schelling, and Schleiermacher, but because it neatly illustrates his philosophy of language. Although some critics of Eric Voegelin’s later work have faulted his failure to deal with the historical Jesus and to address the implications of Christianity for social and political life, the recent publication of Voegelin’s _History of Political Ideas_ has allowed a more complete assessment of his position regarding the Christian political order. This book addresses that criticism through an analysis of Voegelin’s early work. In _Eric Voegelin and the Problem of Christian Political Order_, Jeffrey C. Herndon analyzes the (...) development of Voegelin’s thought regarding the origins of Christianity in the person of Jesus, the development of the church in the works of Paul, and the relationship between an immanent institutional order symbolizing the divine presence and the struggle for social and political order. Focusing on the tension between a spiritual phenomenon based on Pauline faith and the institutionalization of that experience in the church, Herndon offers one of the first examinations of the relationship of the _History of Political Ideas_ to Voegelin’s larger body of work. In his wide-ranging study, Herndon explores Voegelin’s examination of the problem of Christian political order from the inception of Christianity through the Great Reformation. He also presents a clarification of Voegelin’s theory of civilizational foundation and of Voegelin’s philosophy of history with regard to Christianity and Western political order. Herndon addresses not only the nagging problem in Voegelin scholarship regarding his relationship with the historical Jesus but also the “Pauline compromises with the world” that enabled Christianity to become the instrument by which the West was civilized. He also shows that Voegelin’s interpretation of the historical pressures released by the Great Reformation is important to an understanding of his later work regarding the negative effect of Christian symbols in the creation of ideological disorder. _Eric Voegelin and the Problem of Christian Political Order_ clarifies issues in Voegelin studies regarding the intersection between political theory and Christian concerns, addressing the relation of religious experience to the public sphere of political life in the West and helping to explain Voegelin’s contention that the death of the spirit is the price of progress. An empirical study using two ethics-related and three sales force outcome variables was conducted in Taiwan and compared to an existing U.S. sample. Across the two national cultures, individual perceptions of corporate ethics appears to be a more direct determinant of organizational commitment than individual moral values. Differences between the two national cultures were found in ethics perception as it relates to moral values, job satisfaction, and turnover intention. Explanations for the differences are discussed.

Pronouns often convey information about a person’s social identity (e.g., gender). Consequently, pronouns have become a focal point in academic and public debates about whether pronouns should be changed to be more inclusive, such as for people whose identities do not fit current pronoun conventions (e.g., gender non-binary individuals). Here, we make an empirical contribution to these debates by investigating which social identities lay speakers think that pronouns should encode and why. Across four studies, participants were asked to evaluate different (...) types of real and hypothetical pronouns, including binary gender pronouns, race pronouns, and identity-neutral pronouns. We sampled participants from two languages with different pronoun systems: English (N = 1,120) and Turkish (N = 260). English pronouns commonly denote binary gender (e.g., he for men), whereas Turkish pronouns are identity-neutral (e.g., o for anyone). Participants’ reasoning about pronouns reflected both a familiarity preference (i.e., participants preferred the pronoun type used in their language) and—critically—participants’ social ideologies. In both language contexts, participants’ ideological beliefs that social groups are inherently distinct (essentialism) and should be hierarchal (social dominance orientation) predicted relatively greater endorsement of binary gender pronouns and race pronouns. A preregistered experimental study with an English-speaking sample showed that the relationship between ideology and pronoun endorsement is causal: Ideologies shape attitudes toward pronouns. Together, the present research contributes to linguistic and psychological theories concerning how people reason about language and informs policy-relevant questions about whether and how to implement language changes for social purposes. Objectives for ethics education in business school courses generally appear to be based on custom, intuition, and judgment rather than on a more unified theoretical/empirical base. These objectives may be more clearly implemented and their effects studied more rigorously if they could be rooted in the components of ethical decision-making models shown to be influential in ethical decision making. Although detailed studies of code adoption and impact have already been conducted in Hong Kong, there has as yet been no critical analysis of why there has been a gap between the normative and positive factors underlying codes of ethics in Hong Kong. The purpose of this paper is to consider why Hong Kong companies adopting codes of ethics have failed to adhere closely to the best practice prescriptions for code adoption when it would likely be in their best interests (...) to do so. This paper identifies some cultural factors, such as power distance and traditional Legalist assumptions approximating Theory X, that appear to be involved in creating this gap, and offers some practical recommendations for closing the gap, which are presented in the form of hypotheses for further testing. Irrespective of whose fault it really is, much of the blame has been directed at health care providers.Faced with mounting criticism regarding the costs of delivering health care, the industry struck back. Psychological essentialism has played an important role in social psychology, informing influential theories of stereotyping and prejudice as well as questions about wrongdoers’ accountability and their ability to change. In the existing literature, essentialism is often tied to beliefs in shared biology—i.e., the extent to which members of a social group are seen as having the same underlying biological features. Here we investigate the possibility of “value-based essentialism” in which people think of certain social groups in terms of an underlying (...) essence, but that essence is understood as a value. Study 1 explored beliefs about a wide range of social groups and found that both groups with shared biology (e.g., women) and shared values (e.g., hippies) elicited similar general essentialist beliefs relative to more incidental social categories (e.g., English-speakers). In Studies 2-4, participants who read about a group either as being based in biology or in values reported higher general essentialist beliefs compared to a control condition. Because biological essences about social groups have been connected to a number of downstream consequences, we also investigated two test cases concerning value-based essentialism. In Study 3, beliefs about both shared biology and shared values increased inductive generalizations about the social group relative to control, but in Study 4, only the shared biology condition reduced blame for wrongdoing. Together these findings join with recent work to support a broader theoretical framework of essentialism about social groups that can be arrived at through multiple pathways, including, in the present case, shared values. Fair Trade coffee campaigns have improved the lives of small-scale coffee farmers and their families by raising wages, creating direct trade links to farming cooperatives, and providing access to affordable credit and technological assistance. Consumer demand for Fair Trade certified coffee is at an all-time high, yet cooperatives that produce it are only able to sell about half of their crops at the established fair trade price. This article explores the reasons behind this gap between supply and demand and suggests (...) ways to close it. Scholars have long debated the relationship between morality and the market. Some argue that morality tempers market interests, while others argue that the market has its own moral order. Meanwhile, feminist scholars have argued that a false binary between altruism, family, and intimacy on the one hand, and the cold calculus of the market on the other, is based in gender ideologies. Norms around motherhood, in particular, emphasize self-sacrifice, love, and altruism in opposition to self-interested market logics. Commercial surrogacy blurs (...) the line between family and commerce and is therefore an ideal setting for studying tensions between altruism and profit. Drawing on ethnographic research and interviews with 114 actors in the Mexican surrogacy industry, I demonstrate that treating altruism and commercialism as dichotomous can further market interests by preserving the moral palatability and profitability of the industry while perpetuating power asymmetries rooted in gender, race, class, and nationality between surrogate mothers and intended parents.
